Membership Payments
Membership payments are due in May of each year, with reminders being issued for the 2009/2010 year in April. New members are welcome at any time, however full payment is required, as Palliative Care Queensland is unable to offer new or rejoining members pro-rata payments.
New Members
If you would like to become a member of Palliative Care Queensland, please complete a membership application and renewal form. New members are issued with an information pack, and may also be contacted in person by a member of the Palliative Care Queensland team to confirm their contact details.
Existing/ Lapsed Members
If you are a current or former member of Palliative Care Queensland and would like to rejoin as a current financial member for the 2009/10 financial year, please complete a membership application and renewal form.
Membership Payment Options
Telephone: Telephone payments using a credit card can be made during the following hours, Mon, Wed and Fri 9.00am to 4.30pm, by calling Palliative Care Queensland on (07) 3633 0096.
Direct Transfer: Membership payments may be made by transferring the correct amount directly into Palliative Care Queenland’s business account: BSB: 064 012 Account: 1009 4663. Please use the membership name as your payment reference.
Fax: In order to make a payment via fax, please download and complete a membership form and fax to Palliative Care Queensland on (07) 3633 0086.
In-Person: To make a payment in person, please present to the Palliative Care Queensland office during the following times, Mon, Tues, Thurs and Fri 10.00am to 4.00pm.
Mail: In order to make a payment via mail, please download and complete a membership form. Payment can be made via credit card (Visa, Mastercard, AMEX) or cheque/postal order made payable to:
